3. Curriculum Vitae (Europass form) 

4. Transcript rating and earned credits and proof of enrollment at the semester 

5. Learning Agreement for Studies 

6. Proof of citizenship (copy of passport, ID card, or certificate of nationality

and sent scanned to erasmus@mev.hr
